Section 5: Dialysis Interventions for Treatment of AKI

چکیده
Whether or not to provide RRT, and when to start, are two of the fundamental questions facing nephrologists and intensive-care practitioners in most cases of severe AKI. In recent publications, the timing of initiation of RRT was listed as one of the top priorities in research on AKI. However, this dimension has not been included as a factor in any of the large RCTs in this area. The optimal timing of dialysis for AKI is not defined. In current practice, the decision to start RRT is based most often on clinical features of volume overload and biochemical features of solute imbalance (azotemia, hyperkalemia, severe acidosis). However, in the absence of these factors there is generally a tendency to avoid dialysis as long as possible, a thought process that reflects the decisions made for patients with CKD Stage 5. Clinicians tend to delay RRT when they suspect that patients may recover on their own, and because of concern for the well-known risks associated with the RRT procedure, including hypotension, arrhythmia, membrane bioincompatibility, and complications of vascular access and anticoagulant administration. There is also some concern that RRT may compromise recovery of renal function, and increase the progression of CKD. Whether these risks outweigh the potential benefits of earlier initiation of RRT is still unclear.

Different Biomarkers of Acute kidney Injury in Cancer Patients
Acute Kidney Injury (AKI) occurs if the kidneys suddenly lose their ability to remove waste products. When the kidneys lose their ability to filter, dangerous levels of waste products can accumulate, which can upset the chemical composition of the blood and urine. Chemotherapy is one of the methods used to treat or temporarily reduce cancer by using certain medications. The main task of this tr...
متن کاملAcute kidney injury treated with renal replacement therapy and 5-year mortality after myocardial infarction-related cardiogenic shock: a nationwide population-based cohort study.
BACKGROUND Myocardial infarction-related cardiogenic shock is frequently complicated by acute kidney injury. We examined the influence of acute kidney injury treated with renal replacement therapy (AKI-RRT) on risk of chronic dialysis and mortality, and assessed the role of comorbidity in patients with cardiogenic shock. METHODS In this Danish cohort study conducted during 2005-2012, we used ...
متن کاملAcute Kidney Injury: Controversies Revisited
This paper addresses the epidemiology of AKI specifically in relation to recent changes in AKI classification and revisits the controversies regarding the timing of initiation of dialysis and the use of peritoneal dialysis as a renal replacement therapy for AKI. In summary, the new RIFLE/AKIN classifications of AKI have facilitated more uniform diagnosis of AKI and clinically significant risk s...
متن کاملPreoperative renin-angiotensin system inhibitors use linked to reduced acute kidney injury: a systematic review and meta-analysis.
BACKGROUND Previous trials of interventions to prevent acute kidney injury (AKI) have been unsuccessful and additional interventions are needed. Existing reviews of preoperative renin-angiotensin system (RAS) inhibitors have suggested harm. We included more recent studies and conducted this meta-analysis to evaluate the risk of postoperative AKI in patients who received preoperative RAS inhibit...
متن کاملAcute Kidney Injury in the Intensive Care Unit: Timing and Detection
Acute kidney injury (AKI) is common among critically ill patients and results in increased mortality in this population. This review focuses on the diagnosis and management of AKI. The authors first explore new aspects of diagnosis, including new criteria that take into account even modest changes in renal function, and the development of novel biomarkers to allow earlier identification and bet...
